Transforming Care

Our transforming care service provides personalised care and Care for people with learning disabilities, autism and complex needs.

Each individual requires a robust care plan to support progress towards independence, manage risk, and enable them to live happily in their home.

We provide person-led, detailed transition planning to identify bespoke solutions for each person and manage the sensitive process of moving to their new home.

We deliver customised domiciliary care and environments and individual treatment packages to meet complex and interconnected needs.

• People accessing this service will have a learning disability and/or autism and may have mental health conditions or a diagnosis of personality disorder. Many have experienced trauma and display behaviours of concern that can be challenging or risky such as offending, fire setting, and violence and aggression
• The people we work for may also have complex physical health issues including epilepsy and postural issues that require specialist equipment and care.
• Our Carers are recruited for their value base of compassion and respect. Teams work within a psychologically informed practice framework and are trained through the British Institute of Learning Disabilities to deliver positive behaviour Care Plan. This enables us to work safely and respectfully with people and provide an enabling environment where people can grow and reach their potential.
